Louisiana State Licensing Board for Contractors (LSLBC)  In accordance with the LDEQ Air Quality Regulations, LAC 33:III.5151.F.1.f, any contractor performing removal of asbestos containing material that involves Regulated Asbestos Containing Material (see definition in LAC 33:III.5151.B) must become licensed by the Louisiana State Licensing Board for Contractors. You may contact the LSLBC by phone, 225-765-2301 or view their web site for additional information at http://www.LSLBC.louisiana.gov/

Solid Waste Notifications List  (includes Transporters)    (Rev 10/6/08)  A Solid Waste (SW) Transporter # is required to transport any type of solid waste in this state.  This includes any demolition debris and asbestos.  Contact Suzanne Bordelon of the Notifications and Accreditations Section at 225-219-3253 for a number if you don't have one.

Chapter 27:  Asbestos-Containing Materials in Schools and State Buildings Regulation.  Includes provisions for Training Providers and Accreditations (LAC 33:III.Chapter 27).
Chapter 51 (§5151)Emission Standard for Asbestos. Subchapter M.  Includes Asbestos definitions and standards for Demolition and Renovation.   

 Asbestos Accreditation and Notification Forms  

Accreditation Requirements to Ensure Faster Service:  All Asbestos and Lead Accreditation Applications MUST be accompanied with one front face view 1"x 1¼" photograph for each discipline requested.  Also, a check or money order made out to LDEQ must accompany your application.  Please note, LDEQ does not accept cash.   

  • AAC-1:  Asbestos Accreditation Form    (10/1/08) This form is used when applying for all disciplines: Worker, Contractor/Supervisor, Inspector, (Required for schools and state buildings only: Management Planner, Project Designer).
  • AAC-1: Applicacion para Acreditacion de Asbestos  (10/1/08) 
  • AAC-2:  Demolition & Renovation Notification Form     (Revised 8/26/2008)  This form is used any time a demolition occurs whether the structure contains asbestos or not.  If an accredited inspector certifies that there is no asbestos in the structure, complete only the information that pertains to demolition and mark and sign as a Negative Declaration.  This form is also required prior to a demolition or renovation of a structure that contains asbestos and when applying for an ADVF (Asbestos Disposal Verification Form) which must accompany the asbestos being taken to the Louisiana recognized asbestos landfill (in-state or out of state).

AAC-8 Information: For initial asbestos Management Plans or new Management Plans lost in the recent natural disasters.

Note: The revised AAC-8 form is only for initial Asbestos Management Plans. If there is a Management Plan already in place for an existing facility, the Plan just needs to be updated with the 6-month inspections, 3-year inspections (3-year applicable for schools only), and any renovations or additions, etc.  If you no longer have a copy of the asbestos Management Plan as in the case of recent natural disasters, a new Management Plan must be developed using this new form unless you discuss an alternative plan with LDEQ.
